Community Health Worker

This Community Health Worker position is with Phoenix Children's in Phoenix, AZ. Posting Note: We are seeking a personable, adaptable, and compassionate professional to support children and families through care coordination, health education, and resource navigation. This is a new and growing program requiring flexibility and comfort with change. Qualifications Minimum three (3) years of healthcare or related experience Experience with care coordination and resource coordination Knowledge of medical terminology CHW – ADHS Certification required - OR must be able to obtain within 90 Days of start date. Bilingual preferred Skills Strong interpersonal, communication, and interviewing skills Critical thinking and problem-solving abilities Organized with the ability to manage multiple patient records Team-oriented with strong adaptability and flexibility Comfortable providing health education and coaching Proficient with email, Microsoft Office, and patient record systems Additional Details Full-time position Thomas location Standard schedule: 8:00 AM – 5:00 PM, with flexibility as needed A Community Health Worker (CHW) is a nonmedical health worker who serves as a liaison between health, social services, and the community to facilitate access to services, improve the quality and cultural competence of service delivery, and facilitate coordination of services to improve medical and behavioral health outcomes. The Community Health Worker helps eliminate disparities in care to make healthcare systems more equitable by providing health system navigation and resource coordination. By helping with the coordination of care and providing health promotion, this role assists patients with complex conditions and manage office visits, testing services and treatments.
