This Nurse (Clinical Accountability & Metrics) position is with Indian Health Service (Department of Health and Human Services) in Tsaile, AZ. This position is located at Chinle Service Unit, Chinle Comprehensive Health Care Center, Division of Nursing Services, Outpatient Nursing Clinics, Tsaile, AZ. Provides case management & care coordination services for patients, including patient assessments, follow-up care, & evaluation activities to support continuity of care & improve patient outcomes in outpatient clinical settings. Key duties: • Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Coordinates comprehensive case management and care for high-risk and at-risk patients, including assessment, individualized care planning, referrals, follow-up, and evaluation of patient outcomes. Provides skilled direct nursing care across age groups, including triage, emergency treatment, clinical assessment, nursing interventions, and maintenance of accurate health records. Coordinates multidisciplinary services and continuity of care by collaborating with patients, families, providers, specialty services, outside facilities, transportation resources, and other healthcare agencies. Provides patient and family education, counseling, and preventive health support using culturally appropriate approaches to promote self-care, disease prevention, treatment adherence, and improved health outcomes. Supports quality improvement, patient safety, and administrative/community outreach activities through documentation review, performance improvement efforts, staff and community education, and collaboration with the multidisciplinary healthcare team. BASIC REQUIREMENT(S): Education A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education(external link), Council on Accreditation of Nurse Anesthesia Educational Programs(external link), Accreditation Commission for Midwifery Education(external link), or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure- For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level GS-09 in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Specialized experience includes performing professional nursing duties that include providing case management and care coordination for high-risk or at-risk patients; assessing patient needs and developing, implementing, monitoring, and evaluating individualized multidisciplinary plans of care; coordinating referrals, specialty care, follow-up services, and continuity of care with multidisciplinary healthcare teams and outside agencies; providing direct nursing care, triage, and clinical interventions; educating and counseling patients and families on disease processes, prevention, treatment, and self-care; monitoring clinical results and patient outcomes; maintaining accurate clinical documentation; and participating in patient safety and quality improvement activities.
