This Pharmacist (Clinical Pharmacist) position is with Veterans Health Administration (Department of Veterans Affairs) in Prescott, AZ. The Clinical Pharmacist is a licensed professional responsible for providing pharmaceutical care to optimize patient response and resource utilization for professional staff at Bob Stump Department of Veterans Affairs Medical Center in Prescott, Arizona. Key duties: • Typical duties include but are not limited to: Non-VA Prescription Support Receives faxed, mailed, electronically prescribed, or delivered prescriptions written by non-VA providers and documents all received prescriptions in CPRS. Reviews Community Care Referral and Authorization site and CPRS to determine authorization of non-VA Care. Reviews authorized prescriptions for appropriateness using pharmacist knowledge, as well as knowledge concerning the national drug formulary and local VA formulary requirements. Assists in the tracking and shipping of non-VA prescribed controlled substances, as well as all non-VA prescribed medications mailed from the Consolidated Mail-Out Pharmacy (CMOP). Controlled Substance Non-VA Care Prescriptions Reviews the Arizona Prescription Drug Monitoring Program (PDMP) website to ensure appropriateness of received prescription, determine exact due date of medication, and prevent duplicate and overprescribing of controlled substances and document in CPRS. Maintains a current knowledge of national, state, and local laws, regulations, directives, policies, and procedures pertaining to controlled substances. Secures controlled substances while filling Non-VA Care controlled substance prescriptions. Documents all controlled substance entries through the computerized controlled substance package perpetual inventory system. Monitors profiles for abuse potential and notifies the Mental Health or Pain Clinical Pharmacist Practitioner if discovery is made. Telephone Triage Utilizes pharmacy information systems to answer patient or provider questions related to status of their prescription(s), completion date, package tracking, etc. Documents all phone conversations in CPRS for future reference. Functions as a resource person to members of the pharmacy, medical center, and CBOCs regarding non-VA Care Prescriptions. Clinical Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers, as appropriate. Provides patient-specific therapeutic drug monitoring and communicates relevant findings and / or recommendations to other health care providers in charge of the patient both orally and in writing. Evaluates pertinent laboratory data, drug-drug, and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies. Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program. Education Provides education to health care providers regarding medication use. Effectively instructs patients and family members in the appropriate use of medications and medical devices. Maintains basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in practice. Ability to communicate orally and in writing to both patients and health care staff. Possesses knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grade Determinations: GS-12 Clinical Pharmacist Experience or Education: In addition to the basic requirements, candidates must meet one of the following: One (1) year of experience equivalent to the next lower grade level (GS-11) -OR- Completion of an ACPE-accredited Pharm.D. program In addition to the basic requirements, applicants MUST demonstrate the following Knowledge, Skills, and Abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Physical Requirements: The work requires regular and recurring physical exertion involving standing/sitting for prolonged periods of time, lifting/carrying of pharmaceutical supplies weighing up to 50 lbs., and bending/stooping. The work may entail repetitive wrist movement while using various size syringes, vial cap removal and typing. Normal safety precautions and practices are required for working in a healthcare environment.
