This Nurse (Case Management) position is with Indian Health Service (Department of Health and Human Services) in Kayenta, AZ. This position is located at the Kayenta Service Unit in Kayenta, Arizona. This position serves as an inpatient nursing case manager responsible for providing comprehensive case management, utilization management, discharge planning, and care coordination for patients with acute and chronic health conditions and serves as a clinical resource for inpatient services. Key duties: • Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Assess hospitalized patients with complex clinical and psychosocial needs and develop individualized nursing care and discharge plans. Provide direct nursing care, monitor changes in patient condition, and coordinate treatment with the interdisciplinary team. Review admissions and continued stays using approved clinical criteria, and submit documentation needed for payer authorization. Coordinate post-acute services, educate patients and caregivers, and ensure safe transitions from the hospital. Analyze readmissions, avoidable delays, and other case management data to recommend improvements in care coordination. Qualifications: To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). BASIC REQUIREMENT(S): Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Basic Requirements for the GS-11 Grade Level: In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PHD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Your resume must demonstrate at least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Specialized experience includes: Independently assessing and providing nursing care to inpatients with complex health conditions; developing individualized care and discharge plans that address clinical, functional, and psychosocial needs; and coordinating services with patients, families, and interdisciplinary teams. This includes applying approved utilization review criteria to evaluate admission status and continued stay, submitting clinical reviews for payer authorization, arranging post-acute services, educating patients and caregivers, resolving barriers to safe transitions of care, and analyzing case management data to identify opportunities for improvement.
