This Nurse (Clinical Care Coordinator) position is with Indian Health Service (Department of Health and Human Services) in Whiteriver, AZ.
This position is located in the Division of Nursing, Outpatient Departments, Whiteriver Service Unit, Whiteriver, AZ. The incumbent is member of primary medical home care team with both medical provider staff and other health care staff (clinical and non-clinical). A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
Key duties:
• Responsible for setting up and monitoring patients enrolled in disease specific home monitoring and communicating clinical information of changes to clinical plan between patient and provider/care team. Develop and maintain patient registries based on disease, health condition and/or health prevention initiatives. Assist with triage of patients, including triaging telephone calls and walk-in patients. Prepare patients for tests, exams and treatments. Coordinate appointments and procedures for patients who need further care or continuity of care at other facilities. Provide counseling and teaching to patient and families on preventive, curative and rehabilitative measures involved in health care. Qualifications:
To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. BASIC REQUIREMENT(S): Education: A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; OR Your resume must demonstrate at least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples of experience include: Developing a tracking system for patient care coordination and care management across the continuum; Independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; Collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; Provide case management for patients by coordinating services to meet patient's care goals; Provide oversight to ensure compliance and appropriateness of care.
